The tier directly below nursing: one to two years, and — uniquely among the healthcare qualifications here — regulated by the sixteen Bundesländer rather than by federal law. It matters disproportionately, because it is what internationally recruited nurses are actually employed as while their recognition runs, and the nursing file warns about getting stuck there without documenting what “there” is.

Sixteen qualifications, not one
Pflegefachfrau/Pflegefachmann was federalised in 2020: one law, one three-year qualification, one title, valid nationwide. The assistant tier beneath it was left behind.
The result is the most fragmented qualification in this repo. Length ranges from one year to two, the name differs by state, the permitted scope of work differs by state, and a qualification earned in one Bundesland is not automatically usable in another — it may need a state-level equivalence decision to be recognised across a border inside Germany.
This is worse than the Erzieher fragmentation the repo already flags, because it affects the workers with the least capacity to navigate it.
A federal Pflegefachassistenz qualification has been legislated to replace the patchwork with a uniform training of around 18 months, taking effect from 2027. Anything you read about this tier — including this file — should be re-checked against the position in your Bundesland at the time you act. See TODO.md.
What the work is, and where the line is
Assistants do Grundpflege: washing, dressing, mobilisation, feeding, positioning, continence care, observation, documentation, and much of the human contact that a ward’s patients actually experience.
The line above them is drawn in federal law. §4 PflBG reserves to qualified nurses the Vorbehaltsaufgaben: assessing care need, planning and organising the care process, and evaluating its quality. Those may not be delegated, to anyone, at any experience level.
Between the two sits Behandlungspflege — injections, wound care, medication administration, catheter care — which may be delegated to an assistant, but only within what the state qualification covers and what the responsible nurse judges appropriate. A two-year assistant qualification typically carries more of it than a one-year one.
In practice the line moves under staffing pressure. Assistants are routinely asked to do more than their qualification covers. The nurse who delegates it carries the responsibility; the assistant who performs it carries the consequences. Know what your own qualification covers, in your own Bundesland, in writing.
The route
- 1–2 years, mostly at a state-approved school with practical placements
- Entry: usually a Hauptschulabschluss; in some states none at all. It is one of the most accessible qualifications in this repo
- Paid or not — check. Where the training is a proper Ausbildung it is paid; where it is a school course, it may not be
- Above it: a shortened route into the three-year Pflegefachfrau/-mann qualification is available in most states, with the assistant training credited against the first year. This is the important line in this file for anyone planning a career rather than a job
Pay
Market estimate, except for the tariff row.
| Setting | Gross/month |
|---|---|
| Private or non-tariff provider | €2,400 – €2,900 |
| Tariff-bound provider (TVöD-P, lower P-groups) | €2,900 – €3,400 |
| Experienced, two-year qualification, with supplements | €3,100 – €3,600 |
For comparison, a qualified nurse on TVöD-P P7 starts at €3,510 – €3,701 and tops out at €4,305 (verified, valid 01.05.2026 – 31.03.2027). The gap for adjacent work on the same ward is roughly €700–€900 a month, and it widens with experience because the assistant ladder is short.
Supplements matter more here than the base does. Night, Sunday and holiday work is constant in this job and most of the supplement is tax-free — see shift work and supplements. The gap between a tariff employer and a non-tariff one is also unusually wide in care, and the Pflegemindestlohn — a sector minimum set above the general one, with a separate, higher rate for qualified assistants — is the floor beneath all of it. It is a perishable figure and is not quoted here; check the current Pflegekommission rates.
Demand
Effectively unlimited, everywhere, at all times. This is the most readily available employment in this repo. That is not a recommendation in itself: availability this unconditional is a symptom of pay, workload and turnover, not of opportunity.
The trap this file exists for
Internationally recruited nurses are commonly employed as Pflegehilfskraft while their recognition procedure runs. That arrangement is legal, normal and often unavoidable — the Anerkennungspartnerschaft (§16d(3)) is built on it, letting you enter with A2 German and a contract, and complete recognition here.
The risk is structural and worth naming:
Your employer benefits from your being a fully capable nurse who is paid assistant rates. Nothing except their own good faith makes them hurry the recognition.
Reported patterns include the Anpassungslehrgang never quite being scheduled, language courses promised and not funded, shift patterns that make attending the course impossible, and years passing on helper pay.
Protect yourself before you sign:
- Put dates in the contract — when the Anpassungslehrgang starts, how many hours of language course, paid or unpaid, and during working time or not.
- Get the competent authority’s written statement of what you must complete. It is your document, not the employer’s, and it is issued to you.
- Know that the Kenntnisprüfung is an alternative to the Anpassungslehrgang in most states. If the adaptation course is being obstructed, you can ask to sit the exam instead.
- Keep your own record of the clinical work you are actually doing.
- Remember the recognition is yours and portable within the Bundesland — you can change employer without restarting it.
None of this makes the arrangement a bad one. Entering at A2 and qualifying here is a real and well-used route. It goes wrong when nobody sets a deadline.
Foreign-trained candidates
- To work as an assistant: no recognition needed in most states — this is entry-level work, and language plus a willing employer is the practical requirement.
- For a skilled-worker visa: an assistant qualification is generally not enough. §18a requires a recognised vocational qualification of at least two years, so most one-year helper qualifications do not support it. The realistic routes are §16d(3) toward full nursing recognition, §16a to train here, or §26(2) BeschV for Western Balkans nationals.
- A foreign three-year nursing qualification is worth far more than this tier. If you hold one, aim at recognition as a nurse and treat the assistant post as the bridge, with a date on it.
- Language: B1 realistically, A2 to start under an Anerkennungspartnerschaft. Handover and documentation are where it bites.
Pitfalls
- Check the Bundesland. Length, scope, name and portability all change at the state border, and will change again when the federal qualification arrives in 2027.
- Working beyond your qualification is common and is your risk too.
- A one-year qualification supports no skilled-worker visa. Plan for the two-year or the full nursing route if immigration is part of your plan.
- The shortened bridge into full nursing exists in most states and is the single best thing about this tier. Ask about it on day one, not in year three.
- Non-tariff employers dominate the sector. Ask which agreement applies before you compare offers.
