Dispenses, advises and compounds in a pharmacy, under a pharmacist’s supervision — reformed in 2023 to loosen that supervision for experienced staff. Sits under Apotheker the way MFA sits under Arzt, with one important difference: PTA is a regulated profession with a state licence, and MFA is not.

The 2023 reform
The PTA-Reformgesetz took effect on 01.01.2023, replacing the old PharmTAG. What it changed:
- Supervision was loosened. The pharmacy owner may permit a PTA with sufficient experience — broadly, three years — to work with reduced supervision, rather than requiring a pharmacist’s direct oversight of each act. This is the substantive change and it altered what a PTA’s working day looks like.
- The curriculum was strengthened and the internship structure tightened.
- The training remained largely unpaid. This was the profession’s central demand and the reform did not meet it. Two years at a Berufsfachschule, frequently with school fees, and only the six-month pharmacy internship is paid.
That last point is worth stating plainly because it is unusual in this repo: nearly every other qualification here is either a paid dual Ausbildung or a degree with student financing. PTA is neither. It is a school qualification you pay for, for a regulated healthcare profession that pays modestly afterwards — and it is a large part of why entrant numbers have fallen.
What the job involves
- Abgabe und Beratung — dispensing prescriptions, checking interactions, advising on self-medication. The bulk of the work and the reason language matters
- Rezeptur and Defektur — compounding individual preparations and batch stock: ointments, capsules, solutions. German pharmacies still compound, and PTA do most of it
- Prüfung — identity and quality testing of incoming substances
- Abrechnung with the statutory insurers, where a formal error voids reimbursement
- Botendienst, Rezeptur documentation, stock and cold-chain management
- Impfen is not PTA work; vaccination in pharmacies rests with pharmacists
Settings: mostly public pharmacies (Offizin), plus hospital pharmacies, industry and wholesale.
Confusable titles
| Title | Reality |
|---|---|
| Apotheker/in | A university degree plus Approbation, five years. Owns or leads the pharmacy. PTA is not a shortened version of it |
| PKA — Pharmazeutisch-kaufmännische/r Angestellte/r | The commercial assistant: ordering, stock, accounts. A dual Ausbildung, not a regulated profession, and not permitted to dispense |
| Apothekenhelfer/in | The historical name for what is now PKA |
| MFA | The doctor’s-practice equivalent. Unregulated, examined by the Ärztekammer, different work entirely |
Qualification route
- 2 years at a state-approved PTA-Schule — theory, practical pharmacy, chemistry, botany, galenics. Often fee-paying, generally unpaid
- 6 months’ Praktikum in a pharmacy, paid at a modest rate set in the sector agreement
- Staatliche Prüfung, then the Erlaubnis from the state authority
- Entry requirement: mittlerer Schulabschluss
- Above it: Fachweiterbildungen (Onkologie, Ernährungsberatung, Dermokosmetik, Homöopathie), hospital-pharmacy specialisation, or — the significant one — studying Pharmazie to become a pharmacist, for which the PTA qualification confers no exemption but does confer a good reason to be sure
Pay
Covered by the ADEXA/ADA Gehaltstarifvertrag — the same agreement verified for Apotheker in this repo, valid 01.01.2026 – 31.12.2026 and not applying in Nordrhein or Sachsen, which negotiate separately.
The PTA salary groups were not separately verified for this repo — treat the figures below as indicative, not exact, and check the current ADEXA table before relying on them.
| Stage | Gross/month |
|---|---|
| Internship (the 6 months) | Low — a trainee rate, not a wage |
| Entry, newly licensed | €2,600 – €2,900 |
| After several Berufsjahre | €3,000 – €3,600 |
| Experienced with Weiterbildung, or leading Rezeptur | €3,400 – €4,000 |
| Hospital pharmacy under TVöD EG 6–9a | €3,240 – €4,980 |
The hospital row is the verified TVöD VKA scale, valid 01.05.2026 – 31.03.2027, and — as with MFA — the public-sector employer is frequently the better-paying one for comparable work.
Against the pharmacist’s €4,166 rising to €4,922 after eleven years, the shape of the sector is clear: neither role is well paid for its responsibility, and the gap between them is narrower than five years of university would suggest.
Demand and outlook
Shortage of PTA, in a sector that is itself shrinking. Both halves are true and they pull in opposite directions.
- Pharmacies cannot find PTA. Entrant numbers fell — unpaid training in a cost-of-living squeeze is a poor offer — and the shortage is acute.
- But the number of pharmacies is falling. Germany has lost pharmacies steadily for years, squeezed by fixed dispensing fees, mail-order competition and succession failures. A shortage of staff inside a contracting sector does not reliably produce rising pay.
- The reform direction matters to you personally. Proposals to permit branches led by a PTA rather than a present pharmacist recur, are strongly opposed by the pharmacists’ organisations, and would substantially expand PTA responsibility — see Apotheker. Whether the pay would follow is the open question.
Foreign-trained candidates
- Recognition is mandatory. This is a regulated profession; you may not do the work without the state Erlaubnis.
- No automatic EU recognition — PTA is not on the Directive’s list of seven, so even EU qualifications are assessed individually.
- Competent authority is the Bundesland, and procedures differ. See recognition authorities.
- Expect gaps to be found in German-specific content: Rezeptur practice, Apothekenbetriebsordnung, and insurance billing. An Anpassungslehrgang is the usual remedy.
- Language: B2 legally, B2–C1 realistically. Dispensing advice is patient-facing, and getting a dose instruction wrong is a safety event.
- Visa: §18a, or §16d to pursue recognition. No Blue Card — well below the threshold.
Pitfalls
- The training is not paid and may cost you fees. Budget for two years of it. This is the single biggest difference between PTA and almost every other qualification in this repo.
- PTA is not a shortcut to pharmacist. It grants no credit toward the degree.
- Check whether your Bundesland is inside the ADEXA/ADA agreement. Nordrhein and Sachsen are not.
- Reduced supervision is a permission, not a right. Whether you get it depends on the pharmacy owner, and it should be discussed before you accept a post.
- Rezeptur skill is worth money. It is the part of the job least amenable to automation or mail order, and the part where hospital pharmacies compete for you.
