Medizinische/r Fachangestellte/r (MFA) (Medical Assistant)

One of the largest occupations in German healthcare and one of the worst paid relative to what it carries. Examined by the Ärztekammer rather than the IHK or HWK — a chamber system this repo otherwise only meets at the top of the profession — and the reason a doctor’s practice functions at all.

Official title Medizinische/r Fachangestellte/r (MFA)
Former titles Arzthelfer/in — renamed in 2006, still in common speech and older job ads
Protected title No
Category healthcare
Typical qualification Duale Ausbildung, 3 years
Examining body Landesärztekammer — not IHK, not HWK
Regulated No — but delegated medical tasks are legally constrained

A fourth examining body

The repo’s usual picture is three chamber systems: IHK for commercial and industrial occupations, HWK for the trades, Landwirtschaftskammer for the green professions. The MFA Ausbildung belongs to none of them.

It is run and examined by the Ärztekammer — the chamber of the doctors, not of the assistants. The same is true of the dental (ZFA), veterinary (TFA) and pharmacy (PKA) assistant occupations: each is supervised by the chamber of the profession it assists.

This is a practical fact, not a technicality. It decides who assesses a foreign qualification — see recognition authorities — and people lose months applying to IHK FOSA, which is not competent here.

What the job involves

An MFA runs the non-medical half of a practice and a defined slice of the medical half:

  • Front desk and triage — appointments, deciding what is urgent, absorbing the friction when it is not
  • Abrechnung — billing to the Kassenärztliche Vereinigung under EBM, and privately under GOÄ. This is genuinely technical work and errors cost the practice money
  • Delegated clinical tasks — blood draws, injections, ECG, lung function, wound care, dressings, vaccinations where permitted
  • Laboratory and specimen handling
  • Assisting procedures, and sterilisation and hygiene management
  • Radiology where the practice X-rays, which requires a Strahlenschutz certificate

The delegation boundary matters. Medical acts are delegated by the doctor and performed under their responsibility; what may be delegated is defined by professional law and by the doctor’s judgement, and an MFA cannot diagnose or prescribe. Practices vary widely in how much they delegate, which is why two MFA jobs can look like different professions.

Confusable titles

Title Reality
MT / MTA A regulated profession with a state licence, working in laboratories, radiology or functional diagnostics. Higher pay, harder entry. MFA is the primary-care counterpart, not a junior version
ZFA / TFA / PKA The dental, veterinary and pharmacy equivalents. Separate Ausbildungen, separate chambers
Pflegefachfrau/-mann A three-year regulated nursing qualification. Not interchangeable, though practices sometimes employ either
Praxismanager/in A Fortbildung on top of MFA, not a separate entry qualification

Qualification route

  • 3 years dual, in a practice plus Berufsschule. Entry is formally open; most entrants hold a mittlerer Schulabschluss
  • Paid throughout, at rates set in the sector’s own agreement — modest, and below the industrial Ausbildung rates
  • Examined by the Ärztekammer in written and practical parts
  • Above it, and this is where the profession’s real earning potential sits:
Fortbildung What it adds
VERAH / NäPa Delegated home visits and chronic-care management in general practice. A substantive extension of scope
Fachwirt/in für ambulante medizinische Versorgung The Ärztekammer’s Fortbildung at Meister level — practice management
Specialisations Ambulantes Operieren, Onkologie, Dialyse, Hygiene, Impfassistenz, Strahlenschutz
Praxismanager/in, Abrechnungsspezialist/in Administrative and billing leadership

The step to Pflege or to a health-sector Bachelor is also a common route out, and both pay more.

Pay

Market estimate. Pay follows the AAA/vmf Gehaltstarifvertrag, which is an agreement between the doctors’ employer body and the profession’s union — but it binds only employers who have joined, so for most practices it is an orientation rather than an obligation. There is no single binding scale to verify against.

Stage Gross/month
Entry, after the Ausbildung €2,400 – €2,800
Experienced (Tätigkeitsgruppe III–IV, several Berufsjahre) €2,800 – €3,400
With a substantive Fortbildung (VERAH, Onkologie, OP) €3,200 – €3,800
Praxismanagement / Fachwirt, larger practice or MVZ €3,600 – €4,500
MFA employed in a hospital outpatient clinic or MVZ under TVöD EG 5–6: €3,124 – €3,926

That last row is the most useful line in this file. The same person doing broadly the same work earns materially more under a public-sector tariff than in a private practice, with verifiable pay, 30 days’ leave and a pension arrangement attached. Hospital outpatient departments, MVZ, occupational-health services and public health offices all employ MFA on TVöD. Practices compete for staff against that and frequently lose.

The TVöD figures reuse the scale already verified for this repo, valid 01.05.2026 – 31.03.2027.

Demand and outlook

Acute shortage, and it is the practice’s binding constraint. General practices close their books, shorten hours or stop taking new patients because they cannot staff the front desk — not because there is no doctor. Turnover is high, and the exit routes (hospital, industry, insurance, pharma) pay better.

The honest read: demand for MFA is not the problem, and it will not fix the pay. A shortage raises wages where workers can move and employers can pay. Here, practice income is largely set by the statutory fee schedule, so individual practices have limited room — which is exactly why the public-sector and MVZ employers above have become the escape valve.

Direction of travel: more delegation (VERAH, Impfassistenz, telemedicine triage), more MVZ and group practices, and more administrative digitisation — which removes some tasks and adds others.

Foreign-trained candidates

  1. To work: no recognition required. The title is not protected.
  2. For the visa: yes. §18a needs a recognised equivalent qualification, and the competent body is the Ärztekammer, not IHK FOSA. Get that right first.
  3. Equivalence is often partial — German practice billing (EBM/GOÄ), hygiene law and the delegation framework have no equivalent in most systems. Expect an Anpassungsqualifizierung.
  4. Language: B2 realistically, and it is the real gate. The job is telephone, patients in distress, and precise documentation. This is a higher language bar than the pay suggests — the same mismatch as Friseur/in.
  5. Visa: §18a; no Blue Card, well below the threshold.
  6. Strahlenschutz and hygiene certificates are German and do not transfer.

Pitfalls

  • Applying to the wrong chamber is the single most common and most costly mistake.
  • Practice pay varies enormously for identical work, and there is no binding scale to appeal to. Ask which Tätigkeitsgruppe and which Berufsjahre step you are being offered, in those words.
  • Unpaid overtime is endemic in small practices. Your employment rights apply regardless, including the obligation to record working time.
  • The Fortbildungen are where the money is, they are short, and they are frequently employer-funded or fundable — see how retraining is paid for.
  • Do not treat MFA as a stepping stone to medicine. It is not a route into a medical degree, and nothing about it shortens one.
