Logopäde/in und Ergotherapeut/in (Speech and Occupational Therapy)

Two regulated professions written as one file, because their structural story is identical and it is the same one Physiotherapie tells: a state licence, a school-based training that used to charge you for it, pay that does not match the responsibility, and an academisation that has been announced for years and not delivered. Hebamme is what it looks like when it finally is.

Why one file

The repo’s rule is a file per profession, and these two get one between them because a separate file for each would repeat the same seven sections with the clinical content swapped. What differs is the work; what matters for choosing, qualifying, immigrating and getting paid is shared:

  • both are Heilmittelerbringer — therapies prescribed by a doctor and billed to health insurance under the Heilmittel-Richtlinie
  • both are regulated with a state licence and no automatic EU recognition
  • both train for three years in a Berufsfachschule, not a university and not a paid dual Ausbildung
  • both sit on the same unresolved academisation question
  • both pay similarly, and similarly poorly

Where they genuinely diverge, it is marked below.

What the work is

Logopädie — speech, language, voice and swallowing.

  • Dysphagie — swallowing disorders after stroke, in neurodegenerative disease, in intensive care. The most clinically serious part of the field: aspiration kills people
  • Aphasie, Dysarthrie — language and speech after neurological injury
  • Kindersprache — developmental language disorders, articulation, stuttering, cleft palate, hearing-related language development
  • Stimme — voice therapy, including for teachers and singers, and post-laryngectomy
  • Trachealkanülenmanagement in clinical settings

Ergotherapie — restoring the ability to act, in whatever the person’s life requires.

  • Neurologie — after stroke and brain injury: motor function, perception, everyday tasks
  • Geriatrie — maintaining independence, falls, dementia
  • Pädiatrie — sensory integration, motor development, school readiness
  • Psychiatrie — daily structure, work capacity, group therapy. A large field, and the one most often forgotten
  • Handtherapie — post-surgical and post-trauma rehabilitation of the hand, frequently in cooperation with surgeons
  • Arbeitsplatzanpassung and assistive equipment

Confusable titles

Title Reality
Sprachtherapeut/in A wider, partly academic group — Klinische Linguistik, Sprachheilpädagogik, Patholinguistik. They may obtain the same insurance billing approval and do the same work, from a degree rather than the Logopädie school route
Atem-, Sprech- und Stimmlehrer/in Voice and speech training, not a medical profession
Physiotherapeut/in The third Heilmittel profession. Same structure, different body
Ergotherapie vs. Arbeitstherapie Arbeitstherapie is one field within Ergotherapie, not a separate qualification
“Therapeut” alone Unprotected. Anyone may use it

That first row matters more than it looks: Logopädie is one of the few German professions reachable by two structurally different routes, one vocational and one academic, ending at the same practice.

Qualification route

  • 3 years at a state-approved Berufsfachschule, theory plus supervised treatment in the school’s own outpatient clinic
  • Entry: mittlerer Schulabschluss; for Logopädie, most schools test hearing, voice and speech, and a phoniatric certificate of fitness is commonly required
  • Staatliche Prüfung, then the Erlaubnis from the state authority
  • Bachelor routes exist under the Modellklauseln in both professions’ statutes — primärqualifizierend, ending in the same Erlaubnis plus a degree. The clauses have been extended repeatedly rather than resolved
  • Above it: practice ownership, clinical specialisation, Lehrlogopädie and school teaching, academic routes into research and therapy science

Schulgeld — check before you enrol

These professions were notorious for charging students tuition at private schools — several hundred euro a month for a three-year training leading to modest pay, a combination that has no parallel in the paid dual system described in Ausbildung.

Since 2019 the Bundesländer have moved to abolish Schulgeld for the therapy professions, and most have. It is state law, so it is another sixteen-system question — see choosing a Bundesland — and some private schools remain outside the arrangements. Establish for your state and your school what you will pay and whether any training allowance is paid, before signing anything.

The certificate treadmill

Both professions run on post-qualification certificates that are billing permissions as much as skills — they decide what a practice may invoice and at what rate.

Logopädie Ergotherapie
LSVT LOUD, FOTT, Castillo Morales, Trachealkanülenmanagement, Dysphagie-specific courses Bobath, Handtherapie (DAHTH), Sensorische Integration, Affolter, psychiatric method courses

They are frequently self-funded, cost hundreds to thousands of euro each, and consume unpaid leave. Occupational certificates collects the pattern and contrasts it with the employer-funded safety tickets elsewhere in this repo. It is a real, continuing, personally-borne cost that the pay table cannot show — and it is worth asking an employer to fund it, because some do.

Pay

Market estimate, except the public-sector row.

Setting Gross/month
Entry, private practice €2,600 – €3,000
Experienced, private practice €3,000 – €3,600
With sought-after certificates (Dysphagie, Handtherapie, Neuro) €3,300 – €4,000
Clinic / rehab / public sector, TVöD EG 7–9a €3,295 – €4,980
Practice owner Not comparable — a small business

TVöD VKA row verified for this repo, valid 01.05.2026 – 31.03.2027.

One genuine improvement worth recording: Heilmittel prices were substantially raised from 2019, when the TSVG removed the link to regional price ceilings and moved to uniform, higher national prices. Practice revenue rose materially and some of it reached wages. Pay in these professions is better than it was a decade ago — while still sitting below what a three-year regulated healthcare qualification earns in almost any comparable country.

Demand and outlook

Shortage in both, with the sharpest demand in neurology and geriatrics. An ageing population produces stroke, dysphagia and dementia caseloads that only grow, and paediatric waiting lists are long everywhere.

  • Logopädie: dysphagia and neurological rehabilitation are the strongest fields, and clinical dysphagia work is the best-paid and hardest to fill.
  • Ergotherapie: psychiatry and geriatrics are expanding; hand therapy is a well-paid niche with a serious certificate requirement.
  • Practice ownership is genuinely open. Heilmittel practices need a Kassenzulassung based on qualification and premises standards — there is no numerical cap of the kind that rations psychotherapy licences. Compared with almost everything else in this repo’s healthcare section, the path to independence is unobstructed.
  • The academisation question will eventually be decided. If it follows midwifery, the entry bar rises to Abitur and the route becomes a paid dual degree — better for those who come after, disruptive during the change.

Foreign-trained candidates

  1. Recognition is mandatory; the title and the work are both protected.
  2. No automatic EU recognition. Individual equivalence assessment for everyone, including EU qualifications.
  3. Competent authority is the Bundesland. Gaps close through an Anpassungslehrgang or Kenntnisprüfung. See recognition authorities.
  4. Language is the real gate, especially for Logopädie. B2 is the legal floor; the work is in the phonology, grammar and idiom of German, and treating a German child’s articulation or an aphasic adult’s word-finding requires native-level command of the target language. This is the one profession in this repo where the language is not a medium for the work but the object of it — be honest with yourself about it. Ergotherapie is B2–C1 and more forgiving.
  5. Visa: §18a, or §16d to pursue recognition. No Blue Card.
  6. Method certificates do not transfer and will be re-sat here.

Pitfalls

  • Check the Schulgeld position for your Bundesland and your school before enrolling.
  • Logopädie has a second, academic route — if you already hold a linguistics or speech science degree, ask about it rather than starting a three-year school training.
  • The certificates are the career, and they are largely self-funded. Plan and negotiate for them from the start.
  • Practice ownership is reachable, unusually for healthcare here. If independence is the goal, this is one of the few fields where regulation is not what stands in the way.
  • The academisation is unresolved. A Bachelor route may be better or worse for you depending on the state and the school — it is not automatically the higher-status choice yet.
